Photo copies of the following documents are required to process your Contract For Service application:

Photo ID (a valid Driver’s License or Government Photo ID)
Proof of occupancy (a rent receipt, lease agreement, or copy of a warranty deed)
Corporations (a copy of the articles of incorporation)

YOU can upload multiple files at the same time. Drag and drop files or select multiple files with your file/photo manager. Smart device photo uploads are also supported.

Acceptable document types: PDFs
Acceptable photo types: JPGs, JPEGs, PNGs 

Upon successful completion of the uploaded documents/images, select the “Finish” buttton to complete your application.
